The terms of this Notice apply to all records containing your PHI that are created or retained by Triangle Physicians for Women. We reserve the right to revise or amend our Notice of Privacy Practices at any time. Any revision or amendment to this Notice will be effective for all records our practice has created or maintained in the past, and for any of your records we may create or maintain in the future. Our practice will provide a copy of our current Notice upon check in and you may download a copy of it here for your own personal files.
If you believe that you have been discriminated against because of your race, color, national origin, disability, age, sex, or religion in programs or activities that HHS directly operates or to which HHS provides federal financial assistance, you may file a complaint with OCR.
